Step 1. PedsPal participation agreement
- First, complete the PedsPal participation agreement
- Check the boxes of the programs, at the top of page 2, that you wish to receive PedsPal's pricing.
- Vaccine program: The vaccine program is a package. You must choose both Sanofi and Merck.
- If you already have accounts, enter account numbers on the bottom of page 2.
- If you do not have Sanofi and Merck account numbers, you will need to do the following:
- To apply for a Sanofi account, please go to www.vaccineshoppe.com and click on "Register" and follow the instructions.
- To apply for a Merck account, please go to www.merckvaccines.com. Click on "Register" at the top of the screen, then "Begin Registration" and follow the instructions.
Step 2. Become an Office Depot member through PedsPal.
- Complete the portal registration to become an Office Depot member through PedsPal and be linked to our low prices.
Step 3. Participate in our Vaccine Rebate Program.
- If you would like to participate in our Vaccine Rebate Program, please complete this W9 form and fax to 682-885-5860.
Step 4. Henry Schein letter and MedAssets form, plus DEA certificate
- Medical supplies program: The Henry Schein/MedAssets program requires two additional forms, the Henry Schein letter of commitment and the MedAssets participation agreement, as well as a copy of the physician’s DEA certificate. MedAssets is the nation’s largest health care GPO and by having you join MedAssets, Henry Schein can pass their exceptional pricing on to you.
- Without these two forms and a copy of the physician’s DEA certificate, we cannot add you to this contract.
- You do not have to apply for an account number for this program. Once you are linked to our pricing, we will provide you with your Henry Schein account number.
Step 5. Provider sheet
- Fill out the provider sheet.
- When all forms are filled out, fax them to us at 682-885-5860. Don’t forget to fax a copy of your physician’s DEA certificate and the provider sheet.
How long does it take to get PedsPal pricing?
Once the vendors inform us that you are linked to our contracted pricing, we will notify you that you can start ordering.
Approximate time for each vendor to link pricing:
- Sanofi and Merck – 3-7 days
- Henry Schein and MedAssets – 30-45 days
- Office Depot – 48 hours
